You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. Physical Therapy
One-on-one sessions take place in the comfort of your own home with all the We provide all essential therapy equipment for every visit. We accept all forms of payment, including FSA, HSA, Credit, Debit, and Cash payments.
We are in-network providers with Traditional Medicare. For all commercial insurances, we recommend calling the number on the back of your insurance card to obtain out-of-network physical therapy (specialist) benefits. If you have out-of-network benefits coverage, a copay may be required per/visit. Contact us if you have any questions about your insurance/billing.
Golf
Welcome to DRVN Golf.
Our programs increase flexibility, strength, power, balance, and player performance while reducing injury risk. Our licensed Doctors of Physical Therapy and Titleist Performance Institute Certified Medical Level-2 golf experts are here to improve how you play golf! *We provide all of our patients/clients with the "Right To Recieve a Good Faith Estimate of Expected Charges" which is required under the No Surprises Act. Exercise & Weight Loss Programming
Custom exercise programs are tailored to you and your goals. We design custom programs for weight loss, strength training, flexibility, and general healthy living with nutritional guidance. You do not need access to gym equipment for our programs (**if you have access to a full gym, we will tailor your program for the equipment that you have access to).
